Merge pull request #1398 from bnnm/squeak2-etc

- Add more SqueakStream/SqueakSample codecs
- Add SNDS format
- Remove fake extension .sgx (use .sgh+sgd)
- Remove fake extension .tydsp (use .mus)
- Add .trs extension [Kamiwaza (PS2), Shinobido (PS2)]
